The Solution
A Comprehensive Strategic Plan Using Know-Plan-Go
The Emory Centers used our Know-Plan-Go strategic planning process to develop a comprehensive strategic plan for the South Dakota Cardiovascular Collaborative. During the Know phase, we assessed the current and desired state of cardiovascular health in the state through document review, a partner survey, and key informant interviews. Through the Plan phase, we organized and facilitated an in-person strategic planning meeting that enabled key partners to arrive at a consensus on an achievable, evidence-based statewide plan that is inclusive of many heart disease and stroke-related activities conducted by a broad range of partners at the state and community levels. As part of the ongoing Go phase, the Emory Centers supports the Cardiovascular Collaborative in engaging partners to implement the plan, monitoring progress and identifying midcourse corrections, and celebrating achievements.
impact and results
The Know-Plan-Go planning process resulted in a five-year Strategic Plan that includes: Vision, Mission, Guiding Principles, Goals, SMART Objectives, Strategies, and Actions. This Plan provides practical, realistic roadmap for moving forward. The Know-Plan-Go process created buy-in among partners and clarified roles and responsibilities. The Cardiovascular Collaborative has used this plan to guide their work over the past four years, checking in at least annually on progress and make adjustments as needed. As the original five-year plan enters its final year, the Cardiovascular Collaborative plans to work with the Emory Centers and use the Know-Plan-Go process to develop a refreshed five-year plan in 2021.
